Participation on skates is AT YOUR OWN RISK.
Protective gear is mandatory.
This Waiver is only valid for 3 sessions at most, then you need to become a TVR member with the relevant level insurance.
This online form must be completed by all skaters who do not have current Skate NSW membership/Insurance and wish to participate in Tweed Valley Rollers (TVR) training sessions.
Prepayment of the training session ($20) is required. Information on how to do this will be send via email on completion of this waiver.
